KANATA GYMNOSPHERE ONLINE GYMNASTICS/PARKOUR CLASS WAIVER

 

I acknowledge that I am the parent or legal guardian of the child identified above, and I voluntarily authorize such child to participate in online gymnastics/parkour provided by Kanata GymnoSphere.

 

I understand that Kanata GymnoSphere’s online gymnastics/parkour activities will be delivered by a certified gymnastics/parkour coach through an online or virtual platform. I recognize there are aspects of my child’s participation in online gymnastics/parkour activities that are out of Kanata GymnoSphere’s control including but not limited to the safety of the area of which my child is participating in gymnastics/parkour activities and the availability of persons trained in CPR and First Aid. In light of these risks, I agree that I will provide a safe area for my child to participate in online gymnastics/parkour activities that is clear of all obstacles and potential obstacles such as furniture, sharp objects and stairs. The floor of this area must also be padded as appropriate to my child’s age and level of ability such as being carpeted or covered by a gymnastics pad.

 

I further understand that my child is participating in a small group online or virtual gymnastics/parkour activity which means that the Kanata GymnoSphere coach will be providing instruction and coaching to multiple children. I accept that it is my and my child’s responsibility to ensure that my child is visible to the coach through a video feed and can hear the coach at all times while performing gymnastics activities. I understand and agree that my child’s participation in Kanata GymnoSphere’s online gymnastics/parkour activities will take place in common areas of our home such as living room, backyard, family room and not in a bedroom, bathroom or any other private spaces. I further agree that I will directly supervise my child at the beginning of every online gymnastics/parkour activity in which my child participates until class instruction begins. I understand that I am strongly encouraged to supervise my child at all times during my child’s participation in online gymnastics/parkour activities. However, if I choose not to directly supervise my child, I understand and agree that it is my responsibility to be readily accessible to assist my child in the case of injury or emergency occurring during or as a result of participating in online gymnastics/parkour activities.

 

I understand that Kanata GymnoSphere at its discretion may make audio and/or visual recordings of any class session or other online gymnastics/parkour activities and agree that Kanata GymnoSphere has my consent to record either my child’s or my own participation or conversation during such activities.

 

I and my child personally assume all risks whether foreseen or unforeseen in connection with my child’s participation in online gymnastics/parkour activities and my involvement in my child’s participation in online gymnastics activities including but not limited to any and all increased risk of injury due to the activities being conducted online rather than the Kanata GymnoSphere facility. We hereby release, wave, discharge and covenant not to sue Kanata GymnoSphere its officers, trustees, employees, agents and/or members (the released parties) for any claims, injuries, causes of action or liability arising from or in connection with my child’s participation in online gymnastics/parkour activities or my involvement in my child’s participation in online gymnastics/parkour activities provided by Kanata GymnoSphere.

 

I assume complete responsibility for any property damage and/or personal injury caused by me or my child in connection with our participation in online gymnastics activities or my involvement in my child’s participation in online gymnastics/parkour activities.

 

In the event of any accident or injury during or in connection with my child’s participation in Kanata GymnoSphere’s online gymnastics/parkour activities I acknowledge that I am solely responsible for ensuring that my child receives first aid, that my child is transported to a hospital and that medical treatment is initiated as necessary.

 

I have full informed myself of the contents of this assumption of risk, waiver, and release of liability agreement by carefully reading the agreement and seeking any necessary explanation of its terms before signing it. I understand that by signing this agreement I am waving valuable legal rights.
